Some patients just want a better smile. Others are more specific in their requests. "Patients often focus on the front teeth, but in reality the aesthetic zone starts at the second bicuspid or sometimes the first molar depending on how broad the smile is," says Dr. Phillip Elmo of Tuttle Crossing Dental Group.

A smile makeover requires several trips to the dentist. Each procedure uses one or more techniques. Improvements in techniques and materials result in a more natural appearance. General dentistry repairs or orthodontics also may be necessary.

"Upon examination, I may find other conditions of greater dental concern that must be addressed first so I have a solid foundation to do the cosmetic procedures," Elmo says.

Cosmetic procedures

Whitening is the most common cosmetic procedure, while veneers are the primary restorative procedure. Thin, custom-made porcelain or composite laminates are adhered to teeth to adjust length, shape or color. They generally cover the tops and front of teeth.

"The most common combination I see is whitening with veneers on the top front four teeth," Schindler says of the procedures that range from $6,000 to $8,000 at her office.

Bonding repairs chipped or cracked teeth by applying an enamel-like material to the tooth and sculpting it into shape. Laser recontouring reduces excessive gum tissue.

Several options correct the bite and missing teeth. A bridge is a fixed dental appliance bonded to adjacent teeth. A crown replaces missing or damaged portions of a tooth. Implants are artificial roots placed into the bone to support crowns, bridges and dentures.

An American Academy of Cosmetic Dentistry's 2004 survey shows that during the past five years whitening increased 300 percent, bonding increased more than 100 percent and veneers increased more than 250 percent. Credit society's emphasis on youthful appearance and TV shows like Extreme Makeover for the surge.

Cosmetic procedures in highest demand for patients age 20 to 50 are whitening and veneers; patients over 50 sought crowns and bridges, and whitening.

Costs vary depending on the condition of the teeth and the procedures needed. The academy says a comprehensive smile makeover ranges from $10,000 to $25,000, with most practices offering monthly payment plans.
